Śląski Park Technologii Medycznych Kardio-Med Silesia and GABOS SOFTWARE Sp. z o.o. have established cooperation for the implementation of the project co-financed by Regionalny Program Operacyjny Województwa Śląskiego for the period 2014-2020 of the European Social Fund for the priority axis: IX. Social inclusion for the action: 9.2. Available and effective social and health services for sub-measure: 9.2.6. Development of health services – competition.

The goal of the project

Increasing the availability of high-quality health services for 500 people living in the Silesian Voivodeship (especially from medium-sized cities losing socio-economic functions), at risk of social exclusion due to poor health caused by heart failure, by introducing telemedicine services and using them in non-hospital conditions from June 2021 to May 2023.

People with high-energy implant devices will be included in the project (implant. as part of the guaranteed benefits of the NFZ), CRT-D type (resynchronizing stimulator with cardioversion and defibrillation option), and ICD type (cardioverter-defibrillator). The product of the project will be a telemonitoring service, allowing us to monitor the condition of patients with heart failure at home, as a result, this will allow for the deinstitutionalization of forms of support for these people and reduce the factors influencing the social exclusion of the sick.

The main tasks in the project

  • Activation of UP according to the inclusion criteria min. 1 month (and more) after implantation of the implantable device by the medical center/hospital that implements high-energy implantable devices based on medical data (after Echo, ECG, UG, etc. including Barthel UP assessment)
  • UP training in the use of the telemedicine attachment (attachment provided by the Partner, training during activation)
  • Launch of telemonitoring – Centrum Telemedyczne KMS in cooperation with the integration center and Partner responsible for the technological area
  • Constant telemonitoring for 12 months for each UP – and line KMS (paramedic/nurse in consultation with a doctor). The Project Leader and Partner, based on consultations with doctors and implant equipment manufacturers available on the market, have developed the most effective mode of telemonitoring
  • Turning off patients’ device safter 12 months max – visit/consultation with a cardiologist specialist in Zespół Poradni Specjalistycznych KMS (receiving the device, summarizing the results, giving recommendations for further proceedings).
